Waiver & E-mail Registrstion

By selecting the I AGREE option for the Liability Waiver below and completing and sending this registration, I acknowledge that I understand that practicing Yoga is physically strenuous and may create the risk of injury. I agree that I am voluntarily participating in Yoga, with full knowledge of the risk of injury; that I assume that risk; and that Krista Shattes shall not be liable for any injury to me arising out of my participation in such classes or my use of equipment and premises. I agree that neither I, my heirs, assigns, or legal representatives will make any claims or sue Krista Shattes for injury, death, property damage, or loss of any personal belongings, whether or not caused by negligence or otherwise. I have discussed my medical conditions and pre-existing injuries, if any, with my doctor, and he or she told me I can participate in Yoga classes without limitations or restriction. I also understand that photographs and videos may be taken of such classes, and that such photographs and videos may be posted on social media or otherwise for advertising and other purposes. If my likeness should appear in such photographs or videos, I agree that such photographs and videos may be used by Krista Shattes for that purpose without my advance knowledge, and without compensation for use or uses.
